CROMWELL PUBLIC SCHOOLS JOB DESCRIPTION

Position Title: Physical Therapist
Location: District Wide
Position Description: The Physical Therapist (PT) is responsible for assessment, planning, and
collaboration on IEP goal development for students at assigned sites. As a related service
provider, the PT provides appropriate intervention services designed to enhance the student’s
participation and learning potential in a variety of educational environments. Good
communication and interpersonal skills are necessary to collaborate with educators, parents, and
other school support staff. Organizational skills and the ability to work independently are
necessities.
Reports to: Director of Student Services and/or Supervisor of Special Education.
Responsibilities:
● Assess identified students to determine whether there is an educationally significant need
for physical therapy.
● Participate on the Individual Education Plan (IEP) team to establish goals, objectives and
the length of time required to achieve the goals.
● Complete screenings, develop measurable goals, plan and implement interventions
● Schedule and deliver the physical therapy interventions with students as determined in
the IEP.
● Evaluates the physical therapy needs of students using appropriate tests, professional
observations. Shares findings with special education staff and parents.
● Documents all therapy services including daily communication logs, progress summaries,
student Individualized Education Programs (IEP), and Medicaid billing
● Collaborate, consult, and monitor paraprofessionals involved with the implementation of
physical therapy services
● Provide PT services across all four schools, as well as consultation at CCTA
● Be a resource to parents and school staff on the effect of physical limitations affect a
student’s education
● Consult with special education teachers, general education teachers, paraprofessionals,
speech and occupational therapists, physical education teachers, nurses, school
administrators, bus drivers and other school personnel as well as parents to provide
information and recommendations regarding students with physical disabilities.
● Communicate with physicians, and service agencies regarding orthotics and adaptive
equipment needs for the student.
● Participate in and facilitate in-service education, training, and professional development
for related service staff and school personnel as needed.
● Maintain routine inspection of therapy equipment, submit supply orders, and maintain
inventory.
● Adhere to all district health and safety policies.
● Other duties as assigned by the Director of Special Services and building administrator,
which are consistent with the general requirements and qualifications for the position.
Certifications:
● Have successfully completed and graduated from an accredited physical therapy
professional program, have completed all clinical affiliation requirements.
● Two years of experience as a physical therapist, preferably in a school setting
● Requires current State of Connecticut License by the Department of Public Health.
