Transition Coordinator/Special Education Teacher

Job Goals:
To provide support and guidance to students during critical transition periods, such as moving from middle to high school, or high school to post-secondary education
To create and implement individualized education programs (IEPs) to address the unique needs of each student
Performance Responsibilities
Work with individual students to create personalized transition plans based on their needs, strengths, and goals
Facilitate communication and coordination among teachers and staff to ensure a smooth transition for students
Stay informed about best practices in supporting students with diverse needs and attend professional development opportunities to enhance skills and knowledge in the field
Advocate for the rights and needs of students with special needs within the school, district, and community
Modify and adapt curriculum materials and use a variety of teaching strategies to meet the diverse needs and learning styles of students with disabilities
Create a supportive and inclusive classroom environment that promotes positive behavior and addresses the social and emotional needs of students
Collaborate with other professionals, specialists, and families to design IEPs tailored to the unique needs of each student, and to ensure that students receive appropriate resources and support
Conduct ongoing assessments to monitor student progress and adjust instructional strategies accordingly
Maintain accurate and up-to-date records of student progress, IEPs, assessments, and communication with parents and other professionals
Work closely with general education teachers, administrators, and support staff to ensure a cohesive and inclusive educational experience for students with special needs
Stay informed about current research, best practices, and new developments in the field of special education
Perform such other tasks and assume such other responsibilities of a certified professional as assigned by the supervisor
Qualifications
Holds or is eligible for Connecticut State Certification in Special Education (165)
Possesses a degree from an accredited college or university
Possesses extensive knowledge of needs, behavior, and growth patterns of children ages 14-22
Such alternatives to the above qualifications as the Board may find appropriate
Work Year: 187 days
Reports To: Assistant Director of Pupil Services
Terms of Employment: as established by the bargaining agreement between the Board of Education and the Weston Teachers’ Association
