Position: Transition Coordinator
Reports to: Chief Administrator, Education Director
Certification Requirements: Masters level or above with relevant experience in working with transition age students and their families.

Summary:
Serve children with disabilities through program development and coordination of all essential liaison and logistical operations relating to the development of and day to day operations of our transition program. Assists in program administration to ensure quality of special education services, and work with parents, instructional staff, BCBA's and teachers offering appropriate trainings and program monitoring.
Duties and Responsibilities:
- Assist in parent/guardian conferences, development of IEPs, and periodic reviews to assure that appropriate transition services are being provided.
- Provide in service to staff and parents on topics essential for the delivery of services to children with special needs.
- Keep abreast of and disseminate information on educational research, programs, and materials for use with students of transition age within the entire school.
- Develop access to community settings
- Connect with outside agencies who can support employment or provide transition services to share resources
- Support parent in accessing services and completing paperwork required to obtain the services.
- Understand and ensure implementation of the transition bill of rights as required by the SDOE.
- Prepare students to be a participating member of their PPT meeting
- Enter student information into the Special Education Data System (SEDS)
- Job placement in community
- Touring adult programs with parents
- Research appropriate transition assessments
- Support the team in completing comprehensive transition assessments

- Provide appropriate in-service training, workshops, and meetings to keep staff and parents informed of current practices and trends in transition programming
